GP Management Plan-GPMP (Now GPCCMP)

A GP Management Plan (GPMP) is a structured healthcare plan created by a General Practitioner for patients with a chronic or terminal medical condition (lasting 6 months or longer). It outlines the patient’s health goals, care needs, and referrals to allied health professionals.

With a Team Care Arrangement (TCA), patients can receive up to five subsidised allied health visits per year under Medicare.

From 1 July 2025, the GPMP and TCA were replaced by a streamlined single plan, the GP Chronic Condition Management Plan (GPCCMP), under MBS item 965. The core entitlements for patients have not changed, with five subsidised allied health sessions per year. Existing GPMPs will remain valid until 20 June 2027.

Who is eligible for a GP Management Plan?

To qualify for a GPMP (or its replacement, the GPCCMP), a patient must have at least one long-term condition of six months or longer, or a terminal condition. There are no predefined eligible conditions, and the decision is based on the GP’s clinical judgement.

In practice, GP care plans are most commonly prepared for conditions such as:

Cardiovascular Metabolic and endocrine Musculoskeletal 
Heart disease Type 2 diabetes Osteoarthritis 
Chronic heart failure Obesity Rheumatoid arthritis 
Hypertension Thyroid conditions Chronic back pain 
Respiratory Neurological Mental health

COPD Multiple sclerosis Anxiety disorders 
Chronic sinusitis Epilepsy Bipolar disorder 

Importantly, a patient does not need more than one condition to be eligible. A single condition is sufficient if the GP determines that structured management will improve the patient’s care. This applies across all age groups, including children, adults, and the elderly.

Also, the GPMP/GPCCMP is not available for acute or short-term health problems, such as a broken leg or infections. The program is specifically designed for long-term conditions requiring continuous care.

What is a Team Care Arrangement (TCA)? 

A Team Care Arrangement (TCA) is usually prepared alongside a GPMP when a patient needs coordinated care from multiple health professionals. While the GPMP covers the overall care plan, the TCA focuses on collaboration between:

  • The GP 
  • At least two other healthcare providers (e.g., dietitian, psychologist, physiotherapist, podiatrist)

Together, GPMPs and TCAs enable eligible patients to receive Medicare rebates for up to five allied health visits per calendar year under the Chronic Disease Management program.

How to get a GPMP/GPCCMP? 

A GPMP is created during an extended consultation with a GP, as it focuses on assessment and structured care planning rather than a routine visit. It cannot be self-requested online and must be prepared by a GP or a medical practitioner who knows the patient’s medical history.

The process generally includes:

  • Booking a longer appointment: Ask your GP clinic for an extended or care plan appointment. It takes more time than a standard consult. 
  • Discussing your condition: Your GP will review your condition and work with you to establish health and lifestyle goals. 
  • The writing of the plan: Your GP documents your goals, care needs, and which allied health professional you’ll be referred to. They also give you a copy. 
  • Issuing referrals: Your GP provides a referral letter for each type of allied health provider, such as a physiotherapist or dietitian. 
  • Booking your allied health appointments: You may take your referral and Medicare card to your preferred provider. 
  • Tracking your sessions: You can check your remaining sessions in the Medicare app, and the five-session limit renews every 12 months.  
  • Attending regular reviews: Your GP can review the plan every 3 months, and a review after 18 months is required to maintain access to allied health services.
